APPOINTMENTS:
Whenever possible we make appointments at a time to suit you.  With emergencies etc, this is not always possible, and you can help us by:

Plan your appointments ahead of time for regular check ups.
Tell us how many people will be seeing the Dr at the appointment time.
Tell us if you think you may need extra time - insurance medical, minor surgery, counselling
Report to the desk on arrival - we may not always see you walk into the waiting room.
ALL ACC forms, extensions on physio, certificates require an appointment with the Dr
WINZ forms need an appointment with the Dr unless you have been seen in the previous 3 months.
If an emergency arises, it may cause the Dr / Nurse  to run behind schedule. Please try to be tolerant- it may be you who needs the attention some time.  Enquire if you are concerned about any delay.
Our Practice Nurses are available for consultations by appointment - you may occasionally "Catch" them if you do "Pop In" without an appointment , but pre booked consultations do take priority.   It will be of great assistance to the Practice Nurses if you do make an appointment time to see them. 
We have a Practice Nurse available to do cervical smears.
We offer a full vaccination service - childhood & adult.  Some of these require an appointment with both the Dr and the Nurse, some with the Nurse only.  Please enquire when you phone for the appointment.
If you are planning to travel overseas, you will need to make an appointment to see the nurse to go through your itinerary, and plan your vaccine requirements and medications.

RESULTS :
IT IS YOUR RESPONSIBILITY TO CONTACT US FOR YOUR RESULTS
Our Practice Nurses are available to give out your results between 10AM - 4PM
We endeavour to contact you with any abnormal results, but ask that you still ring for your results if have not heard from us.

PRACTICE NURSES
Our Practice Nurses are available for advice and to answer your queries.  We request that you ring between 8.30am - 5.00pm for routine enquiries.
Our nurses also manage and update our recall system for you, sending you reminders about various medical matters. 
They assist your Dr in all aspects of  your confidential care.
If the Practice Nurse is not available when you ring, we will take a message to pass on to them.  Unfortunately they are not always available to take your call as they may be with a patient or assisting the Doctor.
If the Practice Nurse is unavailable, our Reception Staff are not able to give your results. Please do not ask them to do so.
Diabetic Trust Medicals are now to be done by the Practice Nurse. When ringing to make an appointment, please inform Receptionist that this what you require the appointment for.

AFTER HOURS CARE:
We are affiliated to City Doctors, 22 Victoria Avenue, Palmerston North Phone 355 3300, ensuring that you have after hours medical care.
Phoning our surgery prior to 8am and after 5.30pm will direct your call to City Doctors.

PRESCRIPTIONS:
If you need a repeat prescription for your regular / usual medication you may leave a message on our script phone.
There is a surcharge charged for scripts required on the same day as the request.
You MUST leave adequate DETAILS - Name, Strength, Dose of your medication, your DAYTIME contact phone number.
Do not ask questions which need a reply on this phone - if you have a query, you must speak to someone, not leave a message on the tape.
If you phone before 3pm your prescription will ready the next day, if you ring after 3pm, your script will be ready in 2 days time.  If you need urgent medication and have missed the cut off time, you must TALK to someone, not leave a message. Likewise if you have a query about your medication.
Blood pressure and Oral Contraceptive medications require that the nurse check your blood pressure on collection of the script.  You MUST SEE THE DR for every second script for Blood Pressure Medications ie/. 6 monthly. 
If you are over the age of 65, and are on blood pressure or heart medication, you MUST SEE THE DR EVERY TIME you require a new prescription.
For most other medications, you need at least a yearly review with the Dr.  Check with the Practice Nurse who can tell you the frequency of visits you require for your medication.
Ritalin prescriptions require a Dr visit every 3 months.

HOUSE CALLS:
Our Drs are available to do House Calls if you are too unwell to attend our rooms.  The Practice Nurse assesses and arranges for these. They will be done either prior to 9am, at lunchtime, or in the evenings, depending on the Dr's schedule.

ACCOUNTS:
Payment on the day of consultation is preferred. You will receive a $3.00 discount if you pay on the day of consultation.  If you require a credit facility, please speak to our staff.  A statement fee will be charged on all overdue fees. Overdue accounts will be sent to a Debt Collection Agency for recovery. We are happy to receive autopayments if this option suits.

GENERAL:
-         Please hand your consultation slip to reception on your way out. Please check that the details on it are correct. Please make sure we have ALL YOUR CONTACT DETAILS.
-         To enable our Practice to run smoothly, we ask for your assistance.  Please discuss any concerns you may have with regard to our service with Joanne.  We are always happy to look at any improvements / suggestions.  We also have a feedback form in the waiting room and would value your opinions.
-         Please advise reception staff when you change address. This is especially important if you are waiting for an appointment at Palmerston North Hospital or a private specialist.

OUR AIM IS TO PROVIDE YOU & YOUR FAMILY WITH THE BEST POSSIBLE MEDICAL CARE.  PLEASE HELP US TO DO THIS BY VISITING YOUR DOCTOR FOR "WELL HEALTH" CHECKS, AND NOT JUST COMING TO SEE US WHEN YOU ARE UNWELL. PLEASE SPEAK TO OUR PRACTICE NURSES REGARDING "WELL HEALTH" CHECKUPS.
