We aim to improve outcomes and contribute to achievement of a seamless service for our clients exiting prison back into our region. 
A team works together to develop best practice initiatives and develop a process whereby a client�s medical eligibility and their full and correct entitlement is established with referral to the appropriate community services, ensuring consistent care is provided after release.

What happens?

A Prisoner Re-Integration Officer attends one-on-one interviews with clients due to exit from prison.

Clients requesting a medical related benefit will be requested to sign a health consent form to ensure the release of medical information between health practitioners involved in their care and Work and Income.

Prisoner Re-Integration Officer emails a referral to Regional Health Advisor (RHA) advising of Prison release dates.

RHA will liaise with the Prison Medical Staff and Host GPs regarding current medical information and medical review of entitlement if required.

Client will either be medically reviewed in Prison by a medical practitioner or have an appointment scheduled for medical review with their host GP upon release.

The Prison Medical Practitioner will advise of outgoing referrals to community services and back to the clients host GP if they have identified one.

A Work and Income Case Manager and Prisoner Re-Integration Officer will be notified of the recommendation regarding the client's medical eligibility for the most appropriate benefit entitlement.

The Prison Re-Integration Officer will ensure clients benefit entitlements are processed in a timely manner with a referral to the Employment Coordinator if identified in the clients plan.

Follow up will be provided by Work and Income staff as required.
